Thank you for your interest in becoming a member of the Cornplanter Township Volunteer Fire Department. Please read the following instructions carefully before attempting to submit the form.
Download and complete the application for membership and beneficiary form.
Visit the PA Criminal History Background Check website and complete a background check online OR complete this form and submit via mail to the PA State Police. NOTE: This is not a requirement for Junior Members.
Take a clear photo or scan your photo ID or Drivers License.
Junior Members (less than the age of 18) will be required to submit additional documentation: 1) Parent/Guardian Consent Letter, and 2) Working Papers
Once you have obtained all of the above documents, please email to admin@cornplantervfd.com or call (814) 671-8105 to make other arrangements. You may also request hard copies of the application materials.
All parts of the application process must be complete before the application will be reviewed for membership.
Applications will be reviewed and voted on before the general membership during the general membership meeting which is held on the 3rd Tuesday of the month, starting at 7:00 pm. The applicant is required to be present at the meeting when being reviewed for membership.
All applicants will be notified of their membership status once the application has been voted on.
Applications must be turned in to the Board of Directors 30 days prior to the general membership meeting.
Membership dues ($5.00 for adult and junior members) are to be paid at the end of the 180 day probationary period (once the member has been accepted from Probationary to General Member).
